Files not uploading correctly

Lately, images that are being uploaded no longer show a thumbnail, but instead have a DOWNLOAD link. When you download the file, it saves as BLOB with no file extension – if you manually change it to .jpg the file works fine.
It doesn’t happen to all files, but is becoming more frequent.
Any ideas?
