[Plugin: Mad Mimi for WordPress] Plugin is not passing contact info to Madmimi

Hi,

The plugin is no longer passing contact information from the form to the madmimi contact database. Has anyone else had this problem? My account and API have been verified thru the plugin and the entries are being stored on the site. The only thing is that the contact info such as email, first name, last name is not being passed thru.

Any help in resolving this would be appreciated.
