    I don’t think this plug-in will help you with that; you’re going to need to figure out how to style a two column layout within a form. There are probably contact form plug-ins that include built-in layout support for a 2-column layout, but I don’t know of one off hand.

    I think you should be able to use either column shortcodes or some custom markup to split fields into columns. Here`s a guide related to using multiple columns with Contact Form 7. It might come in handy.
