    Can you be more exact what should be in the form? How did you set up it?

    It was a normal plugin setup via the WP dashboard. You can see in my screenshot, what’s in the footer was coming.

    First of all: the label colors should be somehow defied, because it is hard to read them on dark background. I made some CSS hacks but I think it is not the normal way.

    The other fields I can not interpret, maybe try yourself and place the shortcode into a footer widget. As I did. ?? A name, an email address and a submit button would be enough in my case.

    It depends on theme where you use it.

    I see some field doubled in your case, is that you set up that or you have another view somewhere else?

    Well, I see an extra dot, but never mind. But can you customize the form? Remove the “Best contact…” and “City” fields?

    Yes, of course. You can do that in CRM/Settings/Data and Shortcode forms tab.
