Looking for a Theme with Order form

Hi guys,

In general, I am looking for a theme with the main features being the following:

1) Website should contain 2 functioning forms – a simple one and a complex one.

2) User should be able to complete a short simple form which is located on almost any page of the website. On submit, user is redirected to a separate page with more complex form.

3) Details submitted in the simple form, should be automatically transferred to the fields in the complex form. Complex form should contain a variety of fields, which will calculate and estimate the price, depending on what options were chosen in the fields.

That is pretty much the whole functionality needed, anything else is content.
