Cannot configure shipping

I am trying to configure Flat Rate Shipping for my store. I have eight flat rates that vary by the order cost. If the order is 0 – 29.99, shipping in the US is 4.99, 30 – 49.99 it’s 6.99. 50 and over are free.
It’s the same (with different shipping costs) for Canada and International. I’ve watched the set-up video a dozen times and tried every variation of their placeholder calculations and shipping still isn’t being applied nor are the calculators or options showing up on the front end.
