Manage Fields Tab Squished and hard to use

I’ve tried fixing this by deactivating all plugins and changing themes and hasn’t worked. I’m happy to supply FTP and admin info as needed. The layout of the manage fields tab is squished making it nearly unusable to create the forms on the site. Do you have any ideas on how to fix this. A screenshot is attached in the dropbox link. This is time sensitive so don’t know how quickly you can help but the quicker the better. Thanks,
