It looks like you have turned it off for mobile devices. Please edit your opt-in form and go to the “Behavior” step, then open “Display Rules” panel and enable mobile devices.

wow don’t I feel like an idiot.

I had turned it off previously, because it was showing off the screen, but at that time we were running a different theme that was all messed up.
