KYC Pharmacy Form

    I wondered if anyone knew of a plugin that will solve the problem we have.

    We will be selling Pharmaceuticals and for certain products the customer is required by law to fill in a form and answer a few multiple choice questions.

    Is there anything out there that will make this possible?
