Marketpress shipping option not working

I have the premium version of Marketpress commerce plugin (although my WPMU dev subscription has since lapsed) and the shipping option is not working at all.
I have table rates set up to charge for domestic shipping, EU shipping and then rest of world, but nothing is being added to the product prices at checkout.

I deactivated the Marketpress premium version 2.9 and installed Marketpress Lite 2.9.5.6. as a test to see if that version fixes the issue but neither version will add shipping.

Any ideas on a fix?
I can see many other people have had this problem but there is no solution on those posts.
