how to disable email activation on multisite

It was my understanding that the password for new users are given via these emails. How does that work if you disable user email verification? There is no password set when a user makes an account. Also is there a way to add captchas and security questions to avoid spam if there is no email activation?

this plugin does not seem to address these.
