    This article should help you get started.

    Only instead of using the ‘Additional Information’ section, you should use the ‘Location’ section, and add a ‘region’ field under ‘zip’.

    You can see the default array structure for the ‘Location’ section in /admin/class-metaboxes.php and then search for meta_box_fields().
