How do I add more fields on the Resume Submission form?

hi, I would like to know how do I add more fields to the resume form? I want to add gender, date of birth, highest educational level, career level, years of experience, current position, expected salary, last 3 positions held (brief duties)…help!!! ??

I love the plugin by the way!
