mass email problem

Greetings to all,…

I will be doing a mass email to my subscribers list through amazon ses and I would like to know how to attach an opt in form, or link in the email, that relays the info back to my wp sites’ email, so that when a subscriber fills in the form embedded in the email, that info is transmitted back to the email in my site.
Or
How to put a (submit information link) in the email, so that when the recipient clicks on it, an input/opt-in form appears and the information inputted, is transmitted to an email on my site.

I am using Sendblaster, and their support is slow and vague at best.

Any definitive help would be greatly appreciated.
