CAPTCHA not working on any browser

We are trying to implement Web-to-Lead form on our current website. We would like to use your WordPress plugin for this integration and are having 2 separate issues that we need help with.
1. The CAPTCHA images aren’t being displayed. WordPress is installed in the root and we have verified that the FreeType libraries are loaded on the server but the CAPTCHA image is still not loading. Any help would be greatly appreciated.
2. When a name already appears in the database, it redirects to https://www.nfppartners.com/contact-us/index.php rather then just https://www.nfppartners.com/contact-us, the form location.
