I needed a couple more options so I forked your code and added options to change the username and password labels. I only tested the sidebar widget as I’m not using the shortcode form, so that probably hasn’t taken up the new options. I corrected a number of small bugs that showed up given the way I was using the widget, nothing dramatic but worth incorporating into your code.
