Identify the page from where the message was sent

The contact form is part of the footer of my website, so visible on each page.
I’d like to be able to identify from which page the message was sent.
How could I do that?
Would be great to have something like this in the body of the email:—
This e-mail was sent from the contact form on page https://www.xyz.com/biographyIs it possible?
