Installed and activated Fast Secure Contact Forms, now my admin pages are blank!

I used to use this on my old site with no issues, but with latest version of WP whenever I activate the plugin all my admin pages are just blank. I have to rename or delete the plugin folder to restore admin access.
Any ideas?
I’m running 4.1.1 with the Atahualpa theme.
