tick boxes not working

Hi – great plugin, except my tick boxes do not seem to be working on the page. They are displaying (underneath the answer ‘yes’ or ‘no’ and looks like the formatting has jumped around, but are not clickable. I’ve got a feeling it could be clashing with the theme style, but not sure how to fix.
