Radio labels are inconsistent

I have labels and first label of complex fields off.

I have html5 placeholders on.

These are the 3 settings check boxes I have checked. I created a form with a radio option of phone/email, the field is required.

I then duplicated it. Both forms appear on 1 page and 1 form shows the label for the radio buttons, 1 field does not.

My goal is to keep the label on the radio buttons (which it seems that’s the idea if I follow your code correctly.)

Please advise at your earliest convenience. If you have troubles replicating this issue, I can move a version live (currently it is only on my localhost.)
