Multiple Sign Up forms?

  • Hello,
    I was wondering if it is possible to have two instances of the sign form running on the site? For example, I have one page for Health Providers that requires the user to enter their info before seeing that section. I need another sign up form for a Partners section that would do the same thing and send them to different section. Is this possible?
