Supposed to also work for EXISTING images?

    Yes, absolutely. This is what I get even for images uploaded after the plugin was installed:

    <a href="https://domain.com/wp-content/uploads/2015/01/image3.jpg" title="image3" alt="" >
 <img src="https://domain.com/wp-content/uploads/2015/01/image3.jpg" />

    although my options are set twice to override.
    ALT: %name %title
    TITLE: %title image

    My site is on a Multisite installation.
    Does this make a difference whatsoever?
