pdb_signup not working in widget

Hi,
I want to use the signup form in a widget but its not working.
here is a link to the site. its in hebrew but to the right you can see the shortcode. above it theres a link to a page with the shortcode (the page works fine but the widget doesnt work).
how can i fix this?
