Contact Form not sending to designated email address

What is the name of the theme, where did you get it from, and is the contact form widget actually part of theme or is it a separate plugin?

hi, I am using jetpack and I am having similar problem, but email not sending at all to designated email address.
https://www.katherinemhudson.com is site address; page is currently in draft form, I had a published page and was having same problem so I deleted and started from scratch but still same problem. any ideas?

Contact form appears to be a widget that came with the theme.

I have used this theme and moved it live with no problem in the past, so not sure whats happening this time.
