Package/Services Purchase Signup Form

I am currently developing a site that offers marketing services to niche markets. I have come across some issues in not knowing what plugins are best.

I am looking for a plugin where you can design forms that the customer fills out, on completion of the form I wish them to be redirected to the PayPal payment page, them to receive an email from us saying thank you and I would also like an email saying Mr Smith has signed up to package 2 etc.

I have tried Mailchimp but am having issues have also tried WooCommerce however it has more of a product/cart look wish I do not like.

Does anyone know any plugins that achieve this?
