Input Field Displaying Incorrectly

Hi, I’m having trouble with the plugin, specifically getting the input field (“Type the text” box) to display where it is supposed to, specifically on blog post comment forms. It currently is being displayed to the right of where it should be, covering up the Refresh, Audio, and ? buttons. There are also some images that seem out of place, like a border that got repeated or something. Here is a screenshot (MacBookPro, OS X 10.10, Chrome 38.0.2125.111):

Curiously, I have Contact Form 7 installed on the site and it renders just fine on that form.
Dragging the window thinner to kick the page into the mobile view gets the form to look more correct, but still not 100%.

On my actual mobile phone (Nexus 5, Android 4.4.4, Chrome 38.0.2125.114), however, it looks almost the same as on the desktop mobile view, but the box clips out of the text area.

Tried the changing tabindex option, but it changes nothing.
