Contact Form not clearing Name and Email Address after hitting Submit Button

I am using the Monaco theme by Aaviya. The contact form is built in. I am an extreme newbie. Whenever a client puts their name, email address and comment in the form and hits submit, the comment clears, but the name and email address remains. I can go to the admin section and approve, or not approve etc. so that part is working correctly. How can I fix this so when the client comments and hits submit, the form clears. I tried in several browsers and they all are the same. Thank you so much for any help you can give me at. I would be so grateful.
