    Have you tried:

    • flushing any caching plugins you might be running, as well as server and/or browser caches.
    • deactivating all plugins (yes, all) to see if this resolves the problem. If this works, re-activate the plugins one by one until you find the problematic plugin(s). If you can’t get into your admin dashboard, try resetting the plugins folder by FTP or PhpMyAdmin (read “How to deactivate all plugins when you can’t log in to wp-admin” if you need help). Sometimes, an apparently inactive plugin can still cause problems. Also remember to deactivate any plugins in the mu-plugins folder. The easiest way is to rename that folder to mu-plugins-old
    • switching to the Twenty Fourteen theme to rule out any theme-specific problems. If you can’t log in to change themes, you can remove the theme folders via FTP so the only one is twentyfourteen. That will force your site to use it.
    • manually upgrading. When all else fails, download a fresh copy of the latest.zip file of WP (top right on this page) to your computer, and use that to copy up. You may need to delete the wp-admin and wp-includes folders on your server. Read the Manual Update directions first.

    Thanks Mika, I switched off most of the old plug-ins. Now it works !

    My Media uploader looks like this:

    All photos have the same name.

    They need to fix this ASAP. I have customers that pay me for content that I post daily and that includes jpegs. Cmon…

    I’m with vladimir_kovalev: It looks completely the same with me. Every file is uploaded a hundred times. What can I do against it???

    I found it’s the WPML Media plugin cause the media uploader to upload hundreds of times. This plugin is not compatible with WordPress 4.0.

    ok, great information. After deactivating WPML Media, the image upload works properly again, but how can I delete now many hundreds or thousands of empty files, created by WPML Media? Manually this would need many hours our event days…

    disabling plugins worked for me, but now I see an issue where it won’t allow me to save attributes on the image (like opening in separate page). Also no thumbnail on live page. Odd.

    OK – So this is really annoying me now. Like many of others I cannot upload anything to the media library in WordPress 4.0. The progress bar goes all the way and then at the last second it fails and i get the error message asking me to try again later.

    So far I have tried to rectify this by:
    1) Updating all plugins
    2) Deactivating all plugins
    3) Switching to Twenty14 theme
    4) I am not using the WPML plug in which seems to be the cause for many.

    What else can I do? am not a full code junkie and have a reasonable understanding of the back end of my site, but really need some basic level, clear help on this as its stopping me updating my site which I need to do asap.
