Plugin for performance registration

Hi All,
We organize social events and various groups come and perform during the events. Before the event, we need the groups to provide us various data like name of group, contact person, contact number, type of performance ( song / dance / drama etc. ), participants name, performance requirements etc.

Till now we have been collecting this data over email which becomes difficult to manage.

Is there any plugin using which we can generate an online form which groups can access and register online.

Any plugin name or any other pointers would be of great help.
