Plugin for Admin to receive a submission form w/ user dashboard

Hi,

I’m looking for a plugin I can use to allow registered users of my website to fill out and submit a short form (text only) to me.

They then need to be able to have a dashboard where they can view their submissions and it’s status (which I’d like to be able to change from the backend).

I’ve been searching for a while and contempleting going down the route of a private message system between users and myself (admin), using their frontend sent messages as a dashboad; but this would be less than ideal.

It’s to run a contest on my site. thanks in advance for any advice.
