Contact Card

I have been looking for a while and was wondering if there was a plugin or a certain way where you can have your users enter their information and save their input and access anytime later. For instance, I am doing this for my local school band and they need a way that the students can input their name, emergency phone number, and what ever the band directors need for their contact card. Sorry I made this more confusing then it is but I appreciate all help. Thanks!
