More than one form required on site

  • I have multiple lists.
    How do i add additional forms to the site using this plugin.
    In the settings interface, when i select the new list and update the copy, it replaces the previously set up form.

    I want multiple forms on the site for specific individual lists.

    I was not able to see anything of this nature listed in the support logs.
