Is there now any Manual how to create my custom login template?

Copy wp-content/plugins/theme-my-login/templates/login-form.php into your theme directory. Then modify the login template as necessary.

I put in a subfolder with the name theme-my-login (wp-content/templates/mytempl/theme-my-login/?) or just inside tempaltes?

No subfolder, just place the file into your directory – wp-content/templates/mytempl
