Strange : Newly added photos in Media library breaks when added to post

As above. It’s strange as it only started happening recently when the upgrade to 3.8 happen.
If I add a brand new photo to the media library, it’ll be as follows

1) Photo added. Thumbnail can be seen in the media library

https://awesomescreenshot.com/0fb24h1m7b3) But if I click on the broken image, and wait for 2 seconds, the image shows up! ??

Nginx Reverse Proxy on Apache
Cloud VPS server with 2.5GB RAM, 20 GB SSD and 1.2 TB bandwidth.
A Small OrangeMeasures taken that didn’t work yet:
Disabled all plugins, and took all W3TC caches down
Changed theme to twenty fourteen
