Can’t Get PHP Header to Work

I have validated my code and everything is working fine except the one error I get regarding my header. It doesn’t show on my site either. Site is here: https://drbrooker.net I’m at a loss as to how to properly set up this PHP file as an image. I know it’s possible. Any ideas? TIA
