Insert Into Post not working WP3.7.1

Hi… I try to add an image (either changing an existing image or by ‘Add slide’), in either a portfolio or blog post, the Insert Into Post does not work.

I can add images when I choose to ‘Add Multipe images’ but like I say if I try and add a single new slide or if I remove and existing image from the post, then I can not add an image.

I’ve read plenty online with regards to this and I’ve upgraded my site to WP3.7.1 (also upgraded to theme twenty thirteen). I’ve also turned off all my plugins but still can not resolve this issue.
