Keeping your custom theme working with new versions of wordpress

Hi All,

I wanted to see what the process is on how you keep your custom theme up to date and working when wordpress rolls out new versions and changes to their source?

I was looking on google and could not find any info.

If some people could please share their experience, advice and any sites that may help with this topic I would greatly appreciate it.
