COntact form with forced terms of service

My website is https://proactiveitsolutions.ca/ and under “Request Service I have a contact form built using “Easy Contact Forms 1.4.7”
I would like to be able to have users read through a short terms of service and state whether they agree or disagree. How would I go about doing this?
