Plugin Suggestions?

Hello.

I have a stack of papers of doctor bios I need to input to the website. They are all in the same format.
1) Doctor name
2) Specialty
3)… etc .etc etc…

I am looking for a plugin that would allow me to:
1. Set up all these fields in the wp-admin (also allow me to set the title of the field with a h1, h2, etc css class)
2. I would like the fields that I made to show up in EDIT PAGE of wp-admin
3. I would like to be able to fill in the fields with the doctor’s name, their specialty, etc
4. When I click publish, I would like it to look like:

Doctor Name:
Dr. Smith

Specialty:
Family Practice

etc.
etc…

If anyone can help me find a plugin like that, please let me know. I’ve done a ton of google searches and can’t seem to even find the right words to do the search properly.
