TinyMCE Button Missing

I recall using this plugin months ago to make buttons but the icon is missing from the Visual Editor.
Well, after saying that, I looked at a post and saw the button. But in another plugin which uses the Visual Editor the button is missing. I guess it’s just a peculiarity of the plugins.
