Events Manager stopped working correctly

I am helping a friend with his business website, and learning WP as I go.

We are using the Events Manager plugin and all has been going fine until sometime this afternoon. The registration form for the events doesn’t list all of the required fields. It only lists Name, Phone and email. It still error checks for Address, City, State etc.. and therefore doesn’t allow new registrations.

Any ideas?
