1.4.13 prevent to insert media files in posts

After updating to 1.4.13 I was prevented from insert a jpg file in a new post, though it appeared to have been correctly uploaded. Once disactivated the plugin the file finally showed up in the post. Then I was able to activate the plugin again and the image is still there.
