I cannot add media or set a featured image through the "Add media" portal

I’ve been unable to upload images to wordpress since I installed WordPress 3.5.1

Whenever I create/edit a post/page, I cannot add media or set a featured image through the “Add media” portal. When I make an attempt, an error message is prompted once the media file looks like it completed uploading. It doesn’t display images in that page.
But actually file is uploaded in the “media library” section and I can also see the image i attempted to upload in the “media files” section.

I did the following but the issue still persists:

First try deactivating all plugins in case there is a conflict

Temporarily switch to the default Twenty Twelve them then try to upload. still facing problem
