Front-end campaign submition button won't work

Hey Guys! I’m using the shortcode to output the front-end forms to submit a campaign on a custom template page. It looks like the forms output correctly. After everything is filled out, nothing happens when I hit the “submit campaign” button at the bottom. Any ideas for this?
