No WordPress-to-Lead for Salesforce CRM admin page

I’ve installed, uninstalled and reinstalled this plugin. I see Deactivate and Edit links but cannot locate the Configuration page (it was there the first time I installed). I tried going directly to /wp-admin/options-general.php?page=salesforce-wordpress-to-lead and got an insufficient privileges message. However, I’m sure I am logged in as administrator.
