update for simple subscribe

I also would like to be able to disable the category choices in your simple subscriber plug-in. I does me no good to download the previous version as I have no idea where the plug-in folder is located within WordPress. I have done all that I know how to do to find it. I tried to change the code but I got a major error so I had to put it back the way it was. If it displayed the categories in the way I would prefer it would be OK, but it doesn’t, so just a simple box for subscribers to send their email to sign up for email notification when I make a post would be great. Thanks for all your efforts and your plug-in.
