Meta Tag Issue In Post

I manually added meta tags to my blog in header.php section.

the problem here is coming that i want different meta title and meta description for individual post in my blog. But after adding tags to homepage,same meta tags are shown in my posts also.

My question : How can i manually add meta tags to my each post ? i dont want homepage meta tags for my blog post. Please help me out, if i had made any mistake in header.php file then please tell me to fix it
