Can't get my form to work

can i make a form like the one on this page?: https://www.petitetravelers.com/reserve.php

I installed contact form 7 at the location we are moving the site to and I’m tryiing to get that form to function within Contact Form 7. Is this possible? Or do I have to build it from scratch? Where I have the form installed through the contact form code is at https://www.caleblog.com/contact-test/. when you hit send nothing happens.
