Is there a way to wrap the form in a dropdown tab (2tier)?

The situation is like, I have a submission form embedded in my website, which the form consists of 4 members information.

Problem is, I wanted to wrap the team members form field in a dropdown tab, and the whole form wrap into a tab as well.

Which means when I click on the initial tab, a form dropdown together with 3 other tab in it, which consists the other 3 team members form.

And when I click on the team member #2, the form will then dropdown.

Also another problem because these information need to be send in a whole email, so I can track which team members in which team.
