favicon not showing up, read and tried everything I can think of

Hi, I know this has been asked before, and I’ve read the kb article as well as several posts on the subject, but still having same problem:

my favicon.ico is not showing up in the address bar of my browser

Here is one example (I placed code in the head, ico file in the root, cleared cache, nothing):

Any advice?
